The completed report is the most obvious interface between the laboratory and the clinician. An unattractive, uninformative, or confusing report may fail to do justice to an excellent analytical service. Unfortunately, relatively little effort has been expended on achieving the best possible report, and individual enthusiasm and initiative has led to widely diverse practices in reporting. There is an urgent need for standardisation of report formats and it is suggested that all reports should include guidance on abnormality, be in cumulative format, and contain a realistic statement of analytical reliability. This book has been written as a sequence of 'scenes' rather than a formal autobiography. Personal anecdotes and cultural experiences combine to give snapshots of everyday life in the mid-20th century, and insights into the struggles of adapting to life and work in the Middle East. Dr Bold, now 85, has been diagnosed with three unpleasant and life-threatening medical conditions in the past few years. Yet, overall, he considers himself to be a very lucky man. He lives life to the full, spending as much time as possible with family and friends, and indulging his passions for music, gardening and wine.

The completed report is the most obvious interface between the laboratory and the clinician. In-vitro thyroid function tests are difficult to interpret in pregnancy because of, among other things, the effect of oestrogens on thyroid binding globulin (TBG) concentrations. In an attempt to clarify the position, serum concentrations of total thyroxine (T4), free T4, TBG, T4/TBG ratio, tri-iodothyronine (T3) and thyroid stimulating hormone (TSH) were measured. Total T4 and TBG concentrations rose to above the non-pregnant reference range by 20 weeks. The T4/TBG ratio fell to hypo-thyroid values by 20 weeks but although the free T4 level was lower in the third trimester compared with values in the first and second trimesters, only a few subjects had hypothyroid values. The TSH values remained unchanged throughout pregnancy. The significance of these changes is discussed and reference ranges for these hormones at each trimester are provided.
