"Volumes dealing with inborn errors often exceed 1000 pages and present in great detail the biochemical, pathogenetic, clinical, and therapeutic aspects. The pediatric journals, on the other hand, frequently offer single, or at most a cluster of a few, case reports of a rare disorder. Both approaches are discouraging to children's physicians; indeed, it is my impression that many practitioners have "written off" large groups of inherited diseases as hopelessly complex and so rare as to be of no concern to them. This symposium attempts to sift from the mass of detail available the essential data in such a way as to make it useful to doctors caring for young infants, whether neonatologists, family physicians, or pediatricians. To this end, I have asked the contributors to cover pertinent biochemical and other background material, clinical features including presenting signs and usual course, laboratory and other testing necessary to establish the diagnosis, and therapeutic regimens in current use. Disease states clinically manifest and/or requiring attention (such as phenylketonuria) in the first six months of life are discussed. However, because of space limitations, not all inborn errors could be included. Specifically excluded are diseases of hemostasis, cystic fibrosis, inherited sugar transport defects of the intestine, and certain renal transport defects such as vasopressin-resistant diabetes insipidus. In addition to the sections covering specific areas, an article has been included which reviews newborn screening programs. Another proposes a treatment protocol for newborns suspected of having a metabolic disease. Finally, an appendix lists major metabolic treatment centers which have volunteered their services, for information or referrals, to assist the clinician in the care of affected children..." Mary G.
