Lekamwasam was born on June 16, 1959 in Galle, Sri Lanka; the son of Alwis Dias Lekamwasam and Somawathie Kannagara.

Lekamwasam received a Bachelor of Medicine and a Bachelor of Surgery degree from the University of Peradeniya in 1983. Six years later he earned his Doctor of Medicine degree from the University of Colombo.
Lekamwasam began his career as a senior lecturer at the University of Ruhuna in 1989. Since 2004 he has been a professor of medicine at the same university. Sarath is also a former dean Faculty of Medicine at the University of Ruhuna and former president of the Ceylon College of Physicians.
Lekamwasam is a member of the Royal College of Physicians and Osteoporosis Society of Sri Lanka. He is also the chairman of the Board of Study in Medicine at the PGIM.
On October 10, 1988 Sarath Lekamwasam married Vajira Jayasekara. They have two children.
