Hoffman was born in Berrysburg, Pennsylvania on July 21, 1839.
He was awarded the Medal of Honor, for extraordinary heroism shown on April 2, 1865, while serving as a Captain with Company A, 208th Pennsylvania Infantry, at St. Petersburg, Virginia. He died at the age of 65, on April 18, 1905, and was buried at Pomfret Manor Cemetery the in Sunbury, Pennsylvania.
His Medal of Honor was issued on July 19, 1895. The President of the United States of America, in the name of Congress, takes pleasure in presenting the Medal of Honor to Captain Thomas W. Hoffman, United States Army, for extraordinary heroism on 2 April 1865, while serving with Company A, 208th Pennsylvania Infantry, in action at St. Petersburg, Virginia.
