In 1965-1971 he was working as a mechanic at a metallurgical plant in Nizhny Tagil.
In 1971-1972 he was a deputy secretary of the Komsomol committee of the plant.
In 1972-1973 he was the second secretary, and in 1973-1975 – the first secretary of the Nizhny Tagil All Union Leninist Young Communist League.
From August 1975 to June 1977 - the second, and from June 1977 to May 1980 - the first secretary of the Sverdlovsk Regional Committee of the Komsomol. Since 1980 - deputy head of the organizational department of the Sverdlovsk regional committee of the CPSU. In this post he was marked by Boris Yeltsin (then the first secretary of the Sverdlovsk regional committee of the CPSU) and was appointed to the position of an assistant secretary of the regional committee.

From March to October 1988 he worked in the Republic of Afghanistan as an adviser in the apparatus of the Central Committee of the People's Democratic Party of Afghanistan.

In 1991 he took an active part in Yeltsin's presidential election campaign. He left the CPSU after the State Emergency Committee in August 1991, although, according to his own words, he stopped to pay membership fees much earlier.
Since July 1991 - the head of the Presidential Secretariat in the Administration of the President of Russia.
In May 1992, as a result of the reorganization of the President's staff, the secretariat was abolished, and Ilyushin took the post of first assistant of the President. Mikhail Poltoranin testified to his important role while occupying the same position under Yeltsin’s control.
On August 14, 1996 he was appointed to the First Deputy Chairman of the Government of the Russian Federation for Social Policy.
Since September 1996 - Deputy Chairman of the Organizing Committee for the preparation of St. Petersburg for the contest of cities-candidates for the right to host the Games of the XXVIII Olympiad in 2004, the Deputy Chairman of the Government Commission for Operational Issues.

In March 1997, he was removed from office as First Deputy Chairman of the Government of the Russian Federation.

From 1998 - Head of the Department for Work with Regions of the Russian Federation, Member of the Management Board of Open Joint Stock Company Gazprom. He was elected as a deputy of Nizhny Tagil City Council, Sverdlovsk Regional Council, Leninsky District Council of Sverdlovsk.
