John started his career as a surgeon at Dublin Hospital in 1895 - 1897. From 1899 till 1920, he was working in North China as a surgeon to Chinese Government Railways. Since 1902, he was also a surgeon to C. M. Customs in Tientsin and as a health officer at British Municipal Council. From 1916 - 1917 he was working at Queen Alexandra Military Hospital in Millbank and then he was attached to Royal Army Medical Corporations.
