From 1948 to 1950 Ralph J. Miller attended George Washington University. In 1950 he received a Bachelor of Arts degree from the University of Pittsburgh and Doctor of Medicine in 1952.
From 1952 to 1953 Ralph J. Miller was a rotating intern at St. Margaret Memorial Hospital (now UPMC St. Margaret) in Pittsburgh, Pennsylvania. He worked as a resident physician, senior resident physician, and chief resident physician in surgery and urology at Veterans Administration hospitals, university hospitals, and children’s hospital, all in Pittsburgh, between 1953 and 1957. From 1957 to 1960 he was an associate staff member at St. Margaret Memorial Hospital. From 1958 to 1978 Miller served as a senior staff physician at Indiana Hospital, also served as an instructor in nursing, vice-president of medical staff and member of the executive committee.
A urologist, he was in private practice in the Pittsburgh and Indiana areas for more than 45 years, retiring in 2006. In 1975 he was a founder of Indiana Medical Center. In 1978 he was an examining physician at the United States Railroad Retirement Board. In 1979 Miller was an examining physician at the United States Social Security Disability Program. In 1980 Ralph was an aviation medical examiner at the Federal Aviation Administration, and a senior aviation medical examiner in 1984. In 1985 he was a member of Pennsylvania Peer Review Organization. He was a consultant to Social Security Administration.

2000Ralph J. Miller was a member of the International College of Surgeons, Pennsylvania Medical Society, Indiana County Medical Society.
In addition to his professional interests, Ralph J. Miller was accomplished in woodworking and furniture construction.
In 1957 Ralph J. Miller married Joan Masten Paul. They had two children: Ralph J. Miller, Jr. and Mark Paul Miller. She died in 2006.
