Freehafer was born on December 12, 1924 in Philadelphia, Pennsylvania, United States; the son of Fred K. and Mary (Arthur) Freehafer.
Freehafer received a Bachelor of Science degree from Wooster College in 1947. Four years later he earned his Doctor of Medicine degree from Hahnemann Medical College.
Freehafer began his career as an intern at Abington Memorial Hospital (nowadays Abington Hospital-Jefferson Health) in 1951. A year later he took a position of a physician at Hatboro Medical Associates and held it until 1954. Then in 1957, he took a position of a resident at the University Hospitals. Since 1960 Alvin was a physician, specializing in orthopaedic surgery, at Hatboro Medical Associates.
Also he served as an associate professor of orthopaedic surgery at Case Western Reserve University from 1965 to 1979. In 1979, Freehafer became a professor at the same university, where he worked until 1987.
Freehafer was a member of American Academy of Orthopaedic Surgeons, American Medical Association, International Medical Society of Paraplegia, Clinical Orthopaedic Society, American Congress of Rehabilitation Medicine, American Orthopaedic Association and American Society for Surgery of Hand.
On June 28, 1952 Alvin Freehafer married Margaret Irene Gulban. They have 4 children.
